In a genome-wide association (GWA) study, researchers from Boston University Schools of Medicine (BUSM) and Public Health (BUSPH) have identified several genes which influence degeneration of the hippocampus, the part of the brain most associated with Alzheimer disease (AD). The study, which currently appears online as a Rapid Communication in the Annals of Neurology, demonstrates the efficacy of endophenotypes for broadening the understanding of the genetic basis of and pathways leading to AD.

AD is a progressive neurodegenerative disorder for which there are no prevention methods. Available drugs only marginally affect disease severity and progression, making AD effectively untreatable.

GWA studies using very large samples have increased the number of robust associations to 10 genes, including APOE. However, these genes account for no more than 35 percent of the inherited risk of AD and most of the genetic underpinning of the disorder remains unexplained. According to the researchers, magnetic resonance imaging (MRI) of the brain provides in vivo quantitative measures of neurodegenerative and cerebrovascular brain injury that may represent AD-related changes long before clinical symptoms appear. These measures are more powerful than comparisons of individuals with AD with cognitively healthy persons because they avoid misclassification of normal persons who will develop disease in the future.

BUSM researchers conducted a two-stage GWA study for quantitative measures of hippocampal volume (HV), total cerebral volume (TCV) and white matter hyperintensities (WMH). Brain MRI measures of HV, TCV and WMH were obtained from 981 Caucasian and 419 African-American AD cases and their cognitively normal siblings in the MIRAGE (Multi Institutional Research in Alzheimer's Genetic Epidemiology) Study. In addition, similar MRI measures were obtained from 168 AD cases, 336 individuals with mild cognitive impairment and 188 controls (all Caucasian) in the AD Neuroimaging Initiative (ADNI) Study. The MIRAGE Caucasian families and ADNI subjects were included in the first stage and the MIRAGE African American families were added in stage two. Results from the two Caucasians data sets were combined by meta-analysis.

In stage two, one genetic marker (i.e. single nucleotide polymorphism or SNP) from each of the gene regions that were most significantly associated with AD in the Caucasian data sets was evaluated in the African-American data set.

Novel genome-wide significant associations were observed for HV with SNPs in the APOE, F5/SELP, LHFP, and GCFC2 gene regions. All of these associations were supported by evidence in each data set.

"Our two-stage GWAS identified highly significant associations between a measure of degeneration in the brain region most strongly correlated with AD and several genes in both Caucasian and African American samples containing AD, cognitively impaired and cognitively healthy subjects. One of these associations was with the ?4 variant of APOE which is the most well-established genetic risk factor for AD.

Other associations were demonstrated with markers in F5/SELP, LHFP, and GCFC2, genes not previously implicated in this disease" explained senior author Lindsay Farrer, PhD, chief of biomedical genetics at BUSM. He also noted, "previous studies showed that blood level of P-selectin (the protein encoded by SELP) has been correlated with rate of cognitive decline in AD patients."

Farrer believes it is very likely that the number and specificity of these associations will increase in future studies using larger samples and focused on additional precise structural and functional MRI measures. "These findings will inform experiments designed to increase our understanding of disease-causing mechanism and may lead to new therapeutics targets," added Farrer.

Top court upholds healthcare law in Obama triumph

WASHINGTON (Reuters) - The U.S. Supreme Court upheld President Barack Obama's healthcare law on Thursday in an election-year triumph for him and fellow Democrats and a setback for Republican opponents of the most sweeping overhaul since the 1960s of the unwieldy U.S. healthcare system.

In a 5-4 ruling based on the power of Congress to impose taxes, the nation's highest court preserved the law's "individual mandate" requiring that most Americans obtain health insurance by 2014 or pay a tax. The justices also preserved, with some changes, a provision of the law expanding the Medicaid health insurance program for the poor.

Obama and Mitt Romney, the Republican challenger in the November 6 presidential election, immediately responded, with the president calling the ruling "a victory for people all over this country whose lives will be more secure because of this law and the Supreme Court's decision to uphold it."

"We will continue to implement this law and we'll work together to improve on it," said Obama, speaking somberly in the White House East Room, the same setting he used to announce the 2011 death of al Qaeda leader Osama bin Laden.

"What we won't do - what the country can't afford to do - is re-fight the political battles of two years ago or go back to the way things were. With today's announcement, it's time for us to move forward," Obama added.

Romney, who pushed through a similar healthcare overhaul at the state level in 2006 as governor of Massachusetts but opposed Obama's law, called on voters to help him defeat the president in order to repeal the law critics derisively call "Obamacare."

"If we're going get rid of 'Obamacare' we're going to have to replace President Obama. My mission is to make sure we do exactly that," Romney said on the roof of a building overlooking the U.S. Capitol.

Conservative Chief Justice John Roberts was joined by the court's four liberal members - Justices Ruth Bader Ginsburg, Stephen Breyer, Sonia Sotomayor and Elena Kagan - in upholding the law's pivotal "individual mandate" provision.

The four dissenters, all from the court's conservative wing, Justices Antonin Scalia, Anthony Kennedy, Clarence Thomas and Samuel Alito, would have struck down the entire law.

The healthcare law, known formally as the Patient Protection and Affordable Care Act, is the biggest overhaul of the $2.6 trillion healthcare system in about 50 years. It was signed by Obama in March 2010 and promptly put to the test in the courts by 26 of the 50 states and a trade group for small businesses.

The court's decision largely vindicates Obama and Democratic lawmakers in their attempt to fix a system that, while representing 18 percent of the economy, leaves 16 percent of Americans uninsured, a fact that sets the United States apart in the industrialized world.

The U.S. system, unlike other rich countries, is a patchwork of private insurance and restrictive government programs. The United States pays more for healthcare than any other country, but about 50 million of the roughly 310 million Americans still have no insurance at all.

The Obama law was meant to bring coverage to more than 30 million of the uninsured and slow soaring medical costs. Critics said it meddles too much in the lives of individuals and in the business of the states.

'CHARACTERIZED AS A TAX'

Opponents said the individual mandate was an overreach by the federal government and that Congress had exceeded its powers. The court was deeply divided on this issue, but the majority ruled that Congress's taxing authority allowed the mandate.

The law's "requirement that certain individuals pay a financial penalty for not obtaining health insurance may reasonably be characterized as a tax," Roberts wrote for the court's majority.

"Because the Constitution permits such a tax, it is not our role to forbid it, or to pass upon its wisdom or fairness," wrote Roberts.

In another part of the decision, the court said Congress went too far in a part of the law that requires states to expand the government's Medicaid health insurance program for the poor with the goal of covering more of the uninsured.

The court said this problem can be fixed by precluding the federal government from stripping states of existing Medicaid funds if they did not comply with the expansion, and that this did not require striking down other parts of the law.

DRAMATIC DAY AT COURT

The court began its session much as it always does. At 10 a.m. sharp, the marshal's familiar cry of "Oyez! Oyez! Oyez!" rang out as the nine justices entered through heavy scarlet drapes and took their seats at the tall mahogany bench.

The looks on their faces gave nothing away. As he took a seat, Chief Justice Roberts smiled slightly in the direction of the justices' guest seats, where his wife, Jane, was sitting.

As he and other justices read statements, it became obvious that Roberts, appointed by Republican President George W. Bush in 2005, had been the crucial vote upholding Obama's law.

Roberts showed he had taken control over a divided bench. He highlighted flaws in the administration's arguments and stressed the majority was not commenting on the wisdom of the law.

Rather, he said, if it could be upheld, the court must uphold it. He said it was not the job of the judiciary "to save" the nation from policy choices made by elected legislators.

Romney and the Republicans had hoped the Supreme Court would gut the law. Deprived of that outcome, they can now continue pressing the attack on Obama on the campaign trail, but their hopes for a rollback or repeal will hang on legislation, unlikely before the elections, and on the voting public, whose views are mixed.

"What the court did today was say that Obamacare does not violate the Constitution," Romney said. "What they did not do is say that Obamacare is good law, or good policy."

About 56 percent of Americans said they opposed the law in a Reuters/Ipsos poll released on Sunday. When asked about its individual provisions, however, most respondents said they strongly supported them, except for the individual mandate, which was opposed by 61 percent of those surveyed.

Most respondents in the survey favored banning insurers from denying coverage to people with pre-existing conditions; letting young adults stay on their parents' insurance plans until age 26; and making companies with more than 50 workers offer insurance to their employees. All are parts of the law.

So are the creation of state-based exchanges to offer health insurance; insurance premium assistance to poor people; and insurance tax credits for those just above the poverty line.

House of Representatives Speaker John Boehner, the top Republican in Congress, said, "Today's ruling underscores the urgency of repealing this harmful law in its entirety."

Eric Cantor, the No. 2 Republican in the House, said the House would vote on July 11 on legislation that would repeal the law. The Democratic-led Senate is certain to block any repeal legislation coming out of the Republican-controlled House.

Democrats hailed the court ruling as a posthumous victory for a longtime healthcare reform champion, Senator Ted Kennedy, who died in 2009. House Democratic leader Nancy Pelosi said she called his widow Vicki Kennedy to say: "Now, Teddy can rest.

Shares of hospital chains jumped, while large health insurer stocks fell after the ruling. Widening the pool of paying patients stands to benefit hospitals, which are often left to cover the high medical bills of the sick who have no coverage.

WINSTON-SALEM, N.C. June 28, 2012 Taser shots to the chest are no more dangerous than those delivered to other body locations, according to a new study by one of the country's leading experts on the devices.

William P. Bozeman, M.D., an associate professor of emergency medicine at Wake Forest Baptist Medical Center, and colleagues reviewed 1,201 cases of real-life Taser uses by law enforcement agencies but found none in which the devices could be linked to cardiac complications, even when the Taser probes landed on the upper chest area and may have delivered a shock across the heart.

Their findings are published online this month in the Journal of Emergency Medicine.

"While we know that the Taser is a very effective and remarkably safe weapon for law enforcement use, we also continue to have some concern about injuries that may be related to it," Bozeman said. "However, in this review, we found no cases where there was an apparent cardiac complication related to the Taser, even in the worst-case scenarios when the probes landed across the front of the chest and the heart may have been in the path of the electrical current produced by the device."

The team of researchers led by Bozeman reviewed cases of Taser use against criminal suspects by law enforcement officers from agencies across the country. Of the 1,201 cases reviewed, 178, or 22 percent, involved the device's probes landing on the suspect's chest and delivering an electrical charge that may have traveled across the heart area. The majority of the suspects in the reviewed cases were male (94 percent) and the mean age was 32. None of the suspects with Taser shots to the chest were found to have heart-related complications, and they did not have a higher injury rate than suspects with Taser shots elsewhere on the body.

Bozeman said it was important to conduct this research because concerns have been expressed that Taser activations across the front of the chest might be unsafe and should be avoided due to the possibility of probes landing near the heart. It also was important to look at real-life situations, he added, because criminal suspects are thought to be at a higher risk of complications than healthy volunteers because of the stress of the situation, the possible presence of drugs, underlying medical conditions and a variety of other factors.

"This analysis was done to assess whether medical data shows these concerns to be valid and whether law enforcement officers should change how the Taser is used in the field due to possible medical risks," Bozeman said. "The study results indicate that no adverse effects were seen that could be related to transcardiac conduction of Taser shocks."

Tasers are commonly used by law enforcement personnel worldwide as an intermediate-force option to subdue and apprehend potentially dangerous or combative suspects. Tasers function by delivering a series of very brief high-voltage, low-current electric pulses that result in pain, muscle contraction and inhibition of voluntary movement.

Bozeman has been conducting research into the use of Tasers by law enforcement personnel since 2004. In 2007 and 2009 he released results from the first large, independent study of injuries associated with Tasers, finding that they are relatively safe and pose minimal risk of injury. In that study, findings showed that 99.7 percent of those subjected to a Taser had no injuries or only mild ones, such as scrapes and bruises.

In a related study also released in 2009, Bozeman evaluated the immediate cardiac and cardiovascular effects on a group of volunteer police officers, finding that Taser exposure overall was safe and well tolerated. No adverse cardiac affects or rhythm changes were seen.

"This study adds to the accumulated medical evidence that Tasers are safe and effective and that their risks overall are extremely low," Bozeman said. "Although not risk free, Tasers have been associated with lower rates of injuries to suspects and officers than other traditional intermediate force options such as physical force, pepper spray, and handheld impact weapons."

But whether the new service will be a simple rebadge or an entirely new beast remains a mystery. Bloomberg has it on good authority, however, that that Xbox Music will more than just a new face. Microsoft is allegedly combining all the most successful elements of its competitors -- streaming, online storage, and offline syncing -- into a product that will put iTunes, Spotify and Google Play squarely in its crosshairs. The company is allegedly in talks with the record labels to secure the necessary rights for a monthly subscription service and a market for purchasing tracks. It will also take a page from Google Music and allow customers to upload their own collections. If Microsoft can pull off a such a comprehensive service others in the field better watch out -- few companies have the reach or budget of Redmond.

News organizations don't all get ruling right

NEW YORK (AP) ? A rush to quickly report the Supreme Court's decision on President Barack Obama's health care law on Thursday tripped up some news organizations that got it wrong and had to quickly correct themselves.

Both CNN and Fox News Channel initially reported incorrectly that the law's central provision, requiring virtually all Americans to have health insurance, had been struck down. In an apology, CNN said it "regrets that it didn't wait to report out the full and complete opinion regarding the mandate."

The Associated Press got it right, as did other news organizations and broadcast television outlets, generally. A minute after the AP flash alert at 10:07 a.m., The New York Times asked its readers for time, with Washington bureau chief David Leonhardt posting that reporters and editors were analyzing the decision.

"Once we are comfortable with its basic meaning, you can expect a torrent of coverage," he said. The Times next sent news via Twitter at 10:20.

It was a particularly embarrassing muff for CNN, which has suffered through one of its worst ratings quarters in several years, primarily due to a paucity of big news. The network eagerly awaited the Supreme Court's decision Thursday, running a "countdown clock" on its screen during the morning ticking down to 10 a.m.

Anchor Wolf Blitzer and reporter Kate Bolduan quickly reported that the health care law was struck down, based on a reading of Chief Justice John Roberts' decision that the mandate was not a valid exercise of congressional power under the commerce clause. Later in the reading, the justices found other reasons for upholding that portion of the law.

CNN's screen read: "Supreme Ct. Kills Individual Mandate."

"The court striking down that mandate is a dramatic blow to the president," said John King, CNN reporter.

The network also sent an email reporting that the mandate had been struck down and posted the news on Twitter.

By 10:13 a.m., some doubt had seeped in and the onscreen headline read: "Supreme Court Rules on Obama Law."

"Let's take a deep breath and see what the justices actually decided," Blitzer said. "It could be more complicated than we originally thought."

By 10:15, the network was reporting that the entire law had been upheld, and King called it "a huge, huge victory for President Obama."

Fox made the same initial mistake, with Bill Hemmer touting the "breaking news" that the individual mandate had been declared unconstitutional. Fox anchor Bret Baier tweeted the same news. Within two minutes, however, Megyn Kelly was citing the SCOTUSblog in casting doubt on that interpretation, even ordering producers to change an onscreen headline that read: "Supreme Court Finds Health Care Individual Mandate Unconstitutional."

"We're trying to do the best we can," Hemmer said.

Michael Clemente, Fox executive vice president of news and editorial, was unapologetic. "We gave our viewers the news as it happened ... Fox reported the facts, as they came in."

Several members of Congress tweeted incorrect information about the ruling, perhaps based on the initial misreading by cable networks.

Republican Rep. Darrell Issa of California called it a "big win for liberty and the Constitution. Rep. Tom Rooney (R, Fla.) said it was "great news for the American people, victory for constitution."

On CBS, Jan Crawford spoke on the air as she scanned the decision. "It appears, based on our quick summary, that the mandate, the individual mandate at the heart of this case may be in jeopardy under one provision of the constitution but they may have upheld the mandate under another provision."

ABC initially bobbled one detail ? calling the ruling 6-3 instead of 5-4 ? but correctly reported the outcome in a special report by Terry Moran. NBC's Pete Williams said that "the bottom line here is the Supreme Court has upheld the health care law."

Programmable DNA scissors found for bacterial immune system

ScienceDaily (June 28, 2012) ? Genetic engineers and genomics researchers should welcome the news from the Lawrence Berkeley National Laboratory (Berkeley Lab) where an international team of scientists has discovered a new and possibly more effective means of editing genomes. This discovery holds potentially big implications for advanced biofuels and therapeutic drugs, as genetically modified microorganisms, such as bacteria and fungi, are expected to play a key role in the green chemistry production of these and other valuable chemical products.

Jennifer Doudna, a biochemist with Berkeley Lab's Physical Biosciences Division and professor at the University of California (UC) Berkeley, helped lead the team that identified a double-RNA structure responsible for directing a bacterial protein to cleave foreign DNA at specific nucleotide sequences. Furthermore, the research team found that it is possible to program the protein with a single RNA to enable cleavage of essentially any DNA sequence.

"We've discovered the mechanism behind the RNA-guided cleavage of double-stranded DNA that is central to the bacterial acquired immunity system," says Doudna, who holds appointments with UC Berkeley's Department of Molecular and Cell Biology and Department of Chemistry, and is an investigator with the Howard Hughes Medical Institute (HHMI). "Our results could provide genetic engineers with a new and promising alternative to artificial enzymes for gene targeting and genome editing in bacteria and other cell types."

Doudna is one of two corresponding authors of a paper in the journal Science describing this work titled "A programmable dual RNA-guided DNA endonuclease in adaptive bacterial immunity." The second corresponding author is Emmanuelle Charpentier of the Laboratory for Molecular Infection Medicine at Sweden's Ume? University. Other co-authors of the paper were Martin Jinek, Krzysztof Chylinski, Ines Fonfara and Michael Hauer.

Bacterial and archaeon microbes face a never-ending onslaught from viruses and invading circles of nucleic acid known as plasmids. To survive, the microbes deploy an adaptive-type nucleic acid-based immune system that revolves around a genetic element known as CRISPR, which stands for Clustered Regularly Interspaced Short Palindromic Repeats. Through the combination of CRISPRs and associated endonucleases, called CRISPR-associated -- "Cas" -- proteins, bacteria and archaeons are able to utilize small customized crRNA molecules (for CRISPR-derived RNA) to target and destroy the DNA of invading viruses and plasmids.

There are three distinct types of CRISPR/Cas immunity systems. Doudna and her colleagues studied the Type II system which relies exclusively upon one family of endonucleases for the targeting and cleaving of foreign DNA, the Cas9 proteins.

"For the Type II CRISPR/Cas system, we found that crRNA connects via base-pairs with a trans-activating RNA (tracrRNA), to form a two-RNA structure," Doudna says. "These dual RNA molecules (tracrRNA:crRNA) direct Cas9 proteins to introduce double-stranded DNA breaks at specific sites targeted by the crRNA-guide sequence."

Doudna and her colleagues demonstrated that the dual tracrRNA:crRNA molecules can be engineered as a single RNA chimera for site-specific DNA cleavage, opening the door to RNA-programmable genome editing.

"Cas9 binds to the tracrRNA:crRNA complex which in turn directs it to a specific DNA sequence through base-pairing between the crRNA and the target DNA," Doudna says. "Microbes use this elegant mechanism to cleave and destroy viruses and plasmids, but for genome editing, the system could be used to introduce targeted DNA changes into the genome.

Doudna notes that the "beauty of CRISPR loci" is that they can be moved around on plasmids.

"It is well-established that CRISPR systems can be transplanted into heterologous bacterial strains," she says. "Also, there is evidence to suggest that CRISPR loci are horizontally transferred in nature."

Doudna and her colleagues are now in the process of gathering more details on how the RNA-guided cleavage reaction works and testing whether the system will work in eukaryotic organisms including fungi, worms, plants and human cells.

"Although we've not yet demonstrated genome editing, given the mechanism we describe it is now a very real possibility," Doudna says.

This work was funded primarily by the Howard Hughes Medical Institute, the Austrian Science Fund and the Swedish Research Council.

(AP) ? They've known the outcome for three months. Now it's time for the nine Supreme Court justices to share it with the world.

Barring some incredibly strange twist, shortly after 10 o'clock in the morning Thursday, Chief Justice John Roberts is expected to reveal the high court's verdict on President Barack Obama's health care overhaul.

Between 400 and 500 people will crowd into the marble courtroom, the only witnesses to a historic moment that will not be broadcast live on television or radio. Some of those hoping for seats already were in line early Wednesday afternoon.

The court is not just passing judgment on a major expansion of the social safety net, designed to cover an additional 30 million Americans with health insurance. It is doing so on the president's signature domestic legislative achievement and in the heat of his closely fought campaign for re-election.

Four major issues await resolution, the most important of which is whether the law's centerpiece requirement that most people have health insurance or pay a penalty is constitutional. The justices also are weighing whether other parts or indeed the entire 2010 law should fall if they strike down the insurance requirement.

Apart from the insurance mandate, the court also is considering the validity of the health care law's significant expansion of Medicaid, the joint federal-state health care program for the poor.

One wrinkle could prevent the court from deciding the fate of the insurance requirement. A mid-19th century law prohibits challenges to taxes until they have been paid. The insurance requirement does not kick in until 2014 and people who pay the penalty won't do so until they file their 2014 income tax forms by April 15, 2015.

The court devoted more than six hours to arguments about these issues over three days in late March. The justices met March 30 to take a vote on the case and sort out who would take the lead in writing the opinions. In a case of this magnitude, the chief justice probably is writing the majority opinion, assuming he is in the majority.

The 26 states and the small business group challenging the law seemed to have the better of the courtroom arguments in March. Conservative justices peppered Solicitor General Donald Verrilli Jr. with hostile questions about both the insurance requirement and the Medicaid expansion.

But the lively arguments, the only time the justices discuss pending cases in public, are a relatively small part of a rather large record ? in this case, a mountain of more than 140 legal briefs.

Whatever the outcome, it seems certain to become part of the campaign between Obama and his Republican rival, Mitt Romney.

Obama could try to make the court itself, or more particularly its conservative majority, an issue if the insurance requirement or the whole law is struck down. For his part, Romney already has said he will try to take advantage of either outcome. If the law falls, Romney said it will show that "the first 3 1/2 years of this president's term will have been wasted on something that has not helped the American people." If the law is upheld, "we're going to have to have a president ? and I'm that one ? that's going to get rid of Obamacare. We're going to stop it on Day One," Romney said.

The case began almost as soon as Obama signed the law on March 23, 2010. Even before the day was out, Florida and 12 states filed the lawsuit that ended up at the Supreme Court. Another 13 states later joined in later.

The heart of the challenge was the claim that Congress could not force people to buy a product ? health insurance.

The administration advanced several arguments in defense of Congress' authority to require health insurance, including that it falls under the power to regulate interstate commerce.

The government also argued that the insurance requirement was necessary to make effective two other undoubtedly constitutional provisions: the requirements that insurers accept people regardless of existing health problems and limit what they charge older, sicker people.

The administration also said that even if the court rejected the first two arguments, the insurance requirement and penalty are constitutional as an exercise of Congress' power to enact taxes. The penalty assessed for not buying insurance functions like a tax, the government said.
