Short- and mid-term cardiovascular effects of Japan's 2011 earthquake and tsunami: Incidence rises with the seismic peak

ScienceDaily (Aug. 27, 2012) ? The Japanese earthquake and tsunami of 11 March 2011, which hit the north-east coast of Japan with a magnitude of 9.0 on the Richter scale, was one of the largest ocean-trench earthquakes ever recorded in Japan. The tsunami caused huge damage, including 15,861 dead and 3018 missing persons, and, as of 6 June 2012, 388,783 destroyed homes.

Following an investigation of the ambulance records made by doctors in the Miyagi prefecture, close to the epicentre of the earthquake and where the damage was greatest, cardiologist Dr Hiroaki Shimokawa and colleagues from the Tohoku University Graduate School of Medicine at Sendai, Japan, found that the weekly occurrence of five conditions -- heart failure, acute coronary syndrome (including unstable angina and acute MI), stroke, cardio-pulmonary arrest and pneumonia -- all increased sharply soon after the earthquake occurred.

Such reactions -- in ACS, stroke and pulmonary embolism -- have been reported before, said Dr Shimokawa, in Japan, China and the USA. However, these studies reported only the short-term occurrence of individual CVD events, and the mid-term CVD effects of such great earthquakes remain to be elucidated. To this end, the study examined all ambulance transport records in the Miyagi prefecture from 11 February to 30 June for each year from 2008 to 2011 (ie, four weeks before to 16 weeks after 11 March, a total of 124,152 records). Incidence records from before, during and after the earthquake disaster were compared, the aftershocks counted and recorded according to a seismic intensity of 1 or greater.

The number of aftershocks in the Miyagi prefecture was frequent during the six weeks after the earthquake, and the second peak was noted as a large aftershock on 7 April 2011 (magnitude of 7.0). Compared with the previous three years, the significant increases in the occurrence of heart failure and pneumonia were steadily prolonged for more than six weeks after the tsunami struck. On the other hand, the incident increases in stroke and cardio-pulmonary arrest followed the pattern of the first and aftershock seismic peaks. The rapid increases in the occurrence of acute coronary syndromes and cardio-pulmonary arrest was followed by a sharp and significant decline. Interestingly, said Dr Shimokawa, age, sex or residence area did not significantly affect the occurrences of CVD during or following the tsunami.

"To the best of our knowledge," he added, "this is the first report to describe the mid-term course of major cardiovascular events and pneumonia after a great earthquake in a large population. In particular, our findings provide the first evidence that the incidence of heart failure was markedly increased over a long period afterwards." Prevalence of pneumonia, a well known risk factor for deteriorating heart failure, was significantly increased.

The Tohoku University study also found -- as reflected in self-monitoring measurements -- that blood pressure was significantly elevated after the Earthquake. However, transport disruption following the tsunami interrupted delivery of regular medications, such as antihypertensive or antithrombotic drugs, and this may have contributed to the increased cardiovascular events. There was also an increase in the occurrences of ventricular tachyarrhythmias in patients with implantable cardiac defibrillators.

"Taken together," said Dr Shimokawa, "we consider that discontinuation of drugs, activated sympathetic nervous system, rising blood pressure, and the increased occurrence of tachyarrhythmia and infections were all involved in the increased occurrence of cardiovascular events after the Great Earthquake of Japan."

Vegan Coconut Banana muffinsThis post is excerpted from Dreena Burton?s Plant-Powered Kitchen. For the entire article, with lots more tips and photos, go to Vegan Children at School and Packing Plant-Powered Lunches.?Here are some great ideas for lunch meals you can prepare for your children.

Wraps. Wraps are easy for the kids to eat, and can be filled with ?ingredients, such as baked beans, refried beans, chickpeas, lentils and other beans, corn, chopped or grated raw veggies], whole grains, and/or ?chopped leftover baked or roasted potatoes. Also, wraps are a good way to sneak in veggies they might not otherwise eat, plus other nutritional add-ins such as tahini and nutritional yeast. ?Use whole-grain tortillas, nori, or ? if your kids are green leafy stars ? wrap fillings in kale or collard leaves.

Bean Dips ? and yes, Hummus! Typically I make triple (or quadruple) batches of hummus, and freeze one or more containers. Hummus?(more recipes in my books too)?thaws brilliantly ? just takes a little time ? so give this time-saver tip a try. ?Not just for dipping with pitas and ?veggies, hummus can also be used in sandwiches as a spread, in wraps, as a layer on pizzas to hold veggies, or even mixed with grains such as rice or into pasta. Hummus is truly a lunch lifesaver. Make it your friend (and make it yourself; it?s much cheaper and far tastier).

?PB&J?. ?I doubt any schools still allow peanut butter. But some allow nuts, and if not, seeds are usually permitted. So, try different nut and seed butters in sandwiches. ?Since seed butters can be bitter, try adding ground cinnamon to make them naturally sweeter, and a splash of pure maple syrup. dAlso, look into organic soy nut butter spreads? there are even varieties that contain cocoa, for a special treat. Use with whole-grain breads, crisp breads, pitas, tortillas, etc.

Soups and Stews: Soups make a hearty, satisfying lunch for your kids. If you didn?t get to batch-cooking on the weekend, there are some wholesome store-bought alternatives that are also timesavers. I often like to add some cooked beans or whole grains to these soups, to boost the nutritional value. ?I have many hearty soups in my books. ?Some kiddo faves are One While Chick Soup and Mellow Sniffle Lentil Soup from?Eat, Drink, and Be Vegan, and Tomato Lentil Soup with Cumin and Fresh Dill?(doesn?t sound kid-friendly, but is!) and ?Kids? Cheesy Chickpea and White Beans Soup? in Let Them Eat Vegan?(and more examples to follow).

Sauce It Up!: ?Kids love to dip and dunk, so send small containers with lids filled with some of their fave sauces, like tahini or other seed/nut-based sauce, tomato sauce, ?KD Dip?, or a homemade sauce/salad dressing, or maybe a container with nutritional yeast for sprinkling! ?Our girls love nutritional yeast mixed with tahini sauce? whatever works, right? ?And, our girls do not really love pasta (tomato) sauce. ?But, if I add in a splash of balsamic vinegar, they enjoy it much more! ?All these sauces can also be used to mix into whole grains or pasta, along with veggies/seeds/beans/tofu, etc. As I often say ?a sauce makes a meal?!

Grain Bowls: Much like how most vegan restaurants have a ?Buddha Bowl?, you can make something similar for lunches. ?This is where batch-cooking comes in handy. ?Cook more grains than you need for a given meal, so you have extra stored in the fridge during the week. Start with a whole grain/pseudo-grain ? quinoa, millet, brown rice, wild rice, etc ? and add in fave veggies (I use frozen peas a lot ? simply thaw in a bowl of boiled water, then drain and toss in) ? or even fruits like chopped grapes or berries, along with any other nutrient-dense add-ins like hemp seeds, sunflower seeds, pumpkin seeds, beans, tempeh cubes, etc. ?Here is where you can add some of those zippy sauces! ?Pack in a sealed container with a fork/spoon ? read to go!

Pasta with peas by Dreena BurtonPasta Bowls: Much of what you can do with a grain bowl works equally well for leftover pasta. ?I always cook extra pasta (either whole-wheat, kamut, or brown rice), and store in a container in the fridge. ?When making lunches, the pasta will be a little tough/chewy after refrigerating (especially brown rice pasta). ?Simply place in a big bowl, cover with boiled water, then drain after a minute. ?It will soften the pasta again, and also make it more amenable to coating with sauces/seasonings. ?If find the cut shapes (rotini, penne, etc) work best for lunches, easier and neater for little ones to eat than long noodles like spaghetti.

Veggies: ?If you kiddo is really picky about veggies, try grating them fine ? very fine? or mincing in a mini-food processor until almost a pulp, and mixing INTO dips, spreads, nut butters, tofu or chickpea sandwich mashes, or into those grain bowls, pasta dishes, wraps, and more. Yeah, they might notice them ? but they will be too tiny to pick out, and if they like the overall flavor of the dish, they will eat it. ?And, if added to a pasta with other items they love (dried fruit, tamari roasted chickpeas, tofu cubes), they will more likely accept!

"momelet" by dreena burtonBreakfast Foods: Some children love to have some breakfast foods like pancakes, french toast, tofu scrambles, or ?momelets? in their lunchboxes. ?If you prepare a batch or two on the weekend, it can carry you over two or more lunches during the week. ?Momelet?photo by Ricki Heller ? Savory sandwiches. If you have leftover grilled or baked tofu, pulse it through a mini-food processor and then mix in chopped veggies and condiments and use as a sandwich filling. Or simply use slices in sandwiches, with a tahini spread, hummus, or other condiment to hold the tofu in place. Chickpeas can be mashed or pureed and again mixed with veggies and condiments for a hearty sandwich filling, and one that looks not much different from the egg or tuna sandwiches other kids might be eating. Grilled cheese sandwiches are a favorite, and you can use any vegan cheese you prefer, or a nut or seed cheese in place of a commercial vegan cheese. Let cool before packing, and include a small container of ketchup for dipping. If your child is a true veggie lover, make simple sandwiches with chopped, sliced, or grated fresh veggies. ?Use whole grain/sprouted breads, or whole-grain pitas or bagels.

Fruit: Fresh fruit is an obvious choice, especially easy-to-eat fruit such as grapes, strawberries, and orange segments. But others such as cubed melon, plums, kiwi wedges (the kids can eat them like an orange), and half a mango (scored) are welcome changes. Also try replacing raisins with other dried fruit such as apple slices, mango, or apricots. Choose unsulfured and organic whenever possible.

Baked Goods: I am always doing double batches of some variety of muffin, mini-muffin, healthy cookie, snack bar, etc. ?I give many examples at the end of this post of recipes from my books. Coconut Banana Muffins (becoming very popular! See photo at top of post) from Let Them Eat Vegan.

Store-bought items. Lunches and snacks are made even easier with a few prepared items: healthier granola bars, dairy-free yogurts, whole organic fruit bars, unsweetened applesauce cups, crackers, and other more natural snacks. ?And, while foods like Daiya cheese, cheese spreads, and veggie dogs are more processed, there is nothing wrong with incorporating them occasionally, especially on those joyous ?hot lunch? days! I particularly like the Wayfare brand of cheese spread. ?It is oat-based, the ingredients are not from some science experiment, and it works really well for a sandwich/wrap spread to ?hold? other ingredients when you are plum out of hummus! ?(hmmm, plum hummus? plummus?) ?I?ll stop.

Nibbly Bites: ?Kids love finger foods, so things like beans (for our kids, that means Tamari Roasted Chickpeas, Cinnamon Tortilla Strips, Softly Spiced Nuts (if nuts permitted, or you could sub seeds and dried fruit in this recipe), small bites of tofu, tempeh strips?(I simply bake these in the toaster oven on a parchment-lined tray until lightly browned), or seitan, sheets of nori, seaweed snacks, raw crackers, or whole-grain/raw cereals.?Kaia has a line of raw snacks, and some may be just right for your lunchboxes, like ?salt and vinegar pumpkin seeds?!

Local Search for Your Small Business, is it All You Need?

A local hair salon was not getting calls. Their phone was not ringing. New clients were not coming in. A decision was made to try something. Their online local directory listing was claimed, corrected, and optimized. Suddenly their phone started ringing and new customers started coming in. The owner commented that she might have to add more employees to handle the increase in customer traffic. This is a true story.

Local Search MapWhat is Local Search?
Before talking about why you need Local Search let?s define it. According to Wikipedia, Local Search is the use of Search Sites by people searching for local businesses. The search is geographically constrained by adding a location word such as city and state, or zip code.

Wikipedia goes on to say that there are two types of local search. Explicit and implicit. An explicit search is done with a location term. An example of an explicit local search would be shoe stores in raleigh nc. An example of an implicit search would be restaurant. A search for restaurants is implicit because the word is typically used by itself when people are searching for a local restaurant. But how does the search engine know which city to search for with an implicit search? It uses the physical location of the person doing the search. This is usually known by the search site based on the technical information about the computer and it?s connection to the internet.

Local search results are different than non local search results. A local search generates a page that includes businesses from an online business directory. These business directory listings would not show for a non local search. The search engine is making a decision on whether the person is looking for local products or services or not. In the latter case the search could be for research or some other need that does not require a list of local business products or services.

A business directory listing is the basic contact information for a business that exists in a directory. There are many online business directories. Each search engine has one. The online business directory is used for local search and with maps. It is not used with non local search.

When someone clicks on a business directory listing they are taken to the business?s website if the website address has been supplied in the directory listing. If the business has not filled in their own business directory listing with their website address the visitors are taken to the actual business directory information.

Who Needs Local Search?
Back to the main topic about the need for Local Search. If your business sells products or services to local customers, and if your local customers sometimes use the Internet to find what they are looking for, then you need to pay attention to local search.

A Free Opportunity
Local search offers a huge opportunity for businesses and organizations to attract website visitors. It can even help you if you don?t have a website. However, having a website can help your local business directory rank better. A consumer may be simply looking for a phone number for any local flower shop. When searching for flower shops (a local search) that person may see the phone number on the business listing in the search results page and call that number without even clicking to the flower shop website.

Does Your Business have Local Competition?
The challenge that businesses face is the same challenge they face elsewhere. Competition. In this case the challenge is to get your business directory listing ranking above your competitors. The top spot is the best. Each step lower gets fewer clicks, and thus, fewer leads, fewer calls, and fewer customers. If your business directory does not show on page one you won?t see much benefit at all. Few people check listings after page one.

As a business owner you need local search to work for you. You need your business directory listing to show up on the first page of search results for the most common terms that people use when searching for your type of business. You don?t need your website to show on page one when people 800 miles away are searching for local (local for them) products or services.

As an example, if you own an auto body shop you would want your business directory listing to show on page one (page one of search results when someone is searching) when the search term auto body shop in yourcityname is used. You should not try to get your website to show up in search results when someone searches for auto body shop. Of course it might be nice if you could get this to happen. The reality is that it would take a huge amount of time and money. It may be next to impossible. There is little value for you if non local people are finding your website but cannot do business with you.

How to take advantage of Local Search
Now that you know what local search is and why it?s important, you need to know how to make it work for you. Your business must be listed in the major online directories such as Google Places (now called Google + Local), Bing, Yahoo, CitySearch, FourSquare and others.

If your business is at least one year old it may already be listed in many of the online directories. However, you need to claim your listing and fill it in with accurate information. Unclaimed listings contain very little content and often times incomplete and inaccurate information.

Helpful Tools
A helpful tool to get you started is www.getlisted.org. Simply enter your business name and your zip code and it will show you where your business is listed. It will also help you get started by supplying the links to claim your business listing on many important directories.

Another excellent tool is at www.whitespark.ca/local-citation-finder. It helps identify related search terms that people use to search for your type of business. It also finds websites that your business contact information is listed.

Local Search Optimization Tips
Here?s a valuable tip to get you started. Find and claim your Google Plus Local page. Add your business to the directory if you cannot find it. Fill in all the information including your website domain name. Make sure your business contact information is correct and is exactly the same as indicated on your website. Add a review to your Plus Local page.

Local search is important. It?s a huge opportunity offered to you by the search sites. It?s used by a large number of people searching for local products and services. Take advantage of this free gift. It can make a huge difference in your business!

Smoky Pulled Pork Tacos with Cherry-Peach Salsa (Crock Pot ...

While Ben and I have the same views on important matters like family, religion, money management, and hugging, we have to laugh at how opposite we are regarding other parts of our lives.

He loves to skateboard, while I prefer to play tennis.

I?m a coffee drinking early bird, and he?s a hip hop listening night owl.

He hates heights, while I want to jump out of airplanes.

Fly/drive. Summer/winter. Cake/pie. I could go on for days, people!

Fortunately, opposites do, in fact, attract, and while I don?t think we quite knew what to do about these differences in the beginning of our relationship, hysterical laughter is the new norm when proclaiming in coincidental unison our opposite love or hate for a person/place/thing while watching tv, or out-and-about, which happens at least once a day. What else can you do but giggle and agree to disagree?

Luckily I also totally married up and have a husband who will chase after my misdirected tennis balls ?til blinded by sweat, lay with me when I fall asleep at 8pm, fly next to me over oceans, hold my hand in the sweltering heat when I need to hear the cicadas at night, and stuff cake in his face just to make me laugh.

The whole jumping out of an airline bit though? Non negotiable. I can?t get through to the man! ;)

Opposites also tastefully attract in food, which is easily proven by Smoky Pulled Pork Tacos with Cherry-Peach Salsa.

Smoky chipotle pepper-laced pulled pork combines with sweet cherry-peach salsa in a juicy taco that Ben and I both agreed is absolutely mouthwatering. And I quote, ?This is the best thing you?ve made in a long time.? followed by, ?Omg I can?t wait to shove this in my mouth.? regarding his second taco, and ?I?ve never had juicier pork in my life.? Cough,?TWSS, x2. BAHAHAOMG, I can?t even!

These tacos were made even better due to the fact that they were cooked in the crock pot. All I had to do was throw everything in the crocker at night, set it on the base in the morning, then turn it on. The fresh, summery Cherry-Peach Salsa can also be made a couple days ahead of time, which means dinner can be on the table in the time it takes you to shred the pork. Love that!

Start the pulled pork tacos by assembling and prepping the ingredients for the pulled pork.

First slice 1 large onion into quarters, then separate the layers into the bottom of the crock pot. I used a 5 quart crock pot, btw, which fits this recipe beautifully.

Next, address zee butt. A 4-5lb pork butt! Pork butt is actually pork shoulder ? a fact I recently learned after an incredibly confusing conversation between me and my aforementioned husband regarding a requested shoulder rub in which he kept saying, yeah I?ll rub your butt! Right. Moving on!

FIRST cut the pork into 5 or 6 big hunks, then liberally season each piece with salt & pepper. Per the pictures below, I forgot to cut the pork and put it in whole before removing and cutting up. Leaving it whole would have cost me hours in additional cooking time!

Next add a few ingredients that will make the pulled pork taste wonderfully smokey, with a hint of sweet: 4 chipotle peppers in adobo sauce, 2 Tablespoons adobo sauce, 1/2 cup BBQ sauce, 2 Tablespoons brown sugar (un-pictured) and 20oz Dr. Pepper.

Chipotle peppers are smoked jalapeno peppers and are more?smoky?than spicy, in my opinion. You can get cans of them packed in adobo sauce right in the international foods aisle. I generally only use 1 or 2 per recipe, then stash the leftovers in my freezer.

Pop the top onto the crock pot then either stash it in the fridge overnight and place it on the base in the morning, or put it onto the base now and cook on low for 8 hours.

Now, as I mentioned, not only can the pulled pork be prepped in advance, but the sweet Cherry-Peach Salsa that goes on top can be made up to 2 days ahead of time too!

To 1 cup each fresh chopped cherries & peaches, add 1/4 cup chopped cilantro, 1 minced jalapeno, the juice of 1 lime, and a dash of salt.

Mix everything together, then try to save some for the tacos. This is one of my favorite fruit salsas to date, and is seriously delicious on it?s own.

When the pork is cooked through, and fall apart-tender, shred it all up, place it into a bowl, then strain the juices from the crock pot through 2 layers of cheesecloth, and enough of it back in to cover.

Spoon the juicy pork onto a few fajita-sized flour tortillas, top with the cold cherry-peach salsa, then devour!

Smoky Pulled Pork Tacos with Cherry-Peach Salsa

Print this recipe!

Serves 6-8

Pulled Pork Tacos slightly adapted from The Pioneer Woman

Ingredients:

For the pork:

4-5lb pork butt (may be called pork shoulder)

salt & pepper

1 large sweet onion

4 chipotle peppers in adobo sauce + 2 Tablespoons sauce

1/2 cup BBQ sauce

2 Tablespoons brown sugar

20oz Dr. Pepper

Fajita-sized flour quesadillas

For the Cherry-Peach Salsa:

1 cup chopped sweet cherries

1 cup chopped peaches (about 1 large peach)

1/4 cup chopped cilantro

1 jalapeno, minced

salt

juice of 1 lime

Directions:

1. Cut onion into quarters then separate layers into the bottom of a 5 quart (minimum size) crock pot. Trim pork butt of excess fat then cut into 4-6 large pieces and season well with salt & pepper. Lay seasoned pork on top of onions then add chipotle peppers, adobo sauce, BBQ sauce, brown sugar, and Dr. Pepper. Cover and cook on low for 8 hours or until pork is tender enough to easily shred with a fork.

2. Remove pork pieces from the crock pot and shred, then place into a bowl and set aside. Strain juices through a double layer of cheesecloth and add back to shredded pork. Serve with Cherry-Peach Salsa in flour tortillas.

3. For the Cherry-Peach Salsa: Combine all ingredients in a bowl and mix well. Make up to two days in advance.

I was afraid the pork would be too sweet with the addition of BBQ sauce, Dr. Pepper and brown sugar, but it was perfect. Definitely not too sweet, because, wouldn?t you know it ? while I love sweet & savory dinners, Ben?s all savory, all the time. Sweets are for dessert! Which he also doesn?t like. Come on! ;)

I also originally wanted to make just a cherry salsa to top the tacos with, as a wink and a nod to Cherry Dr. Pepper, but I?m so glad I threw chopped peaches in there too. Their scent and flavor is just outrageous right now, and guess what? Even though Ben?s a savory guy, he LOVED the fruity salsa. That?s a major win, people!

Knock the chipotle peppers and adobo sauce down a bit if you are sensitive to heat but as is, the heat level is only about a 5/10, and the cherry-peach salsa helped tone it down too!

While we definitely have a lot of opposite interests, Ben and I like driving in the country, planning for our future, cooking, and going out to eat together!
